What it is
The deep plane facelift is a rhytidectomy technique in which the surgeon works beneath the SMAS, the musculo-aponeurotic layer that supports the tissues of the face. Instead of pulling only the skin, the technique releases the retaining ligaments and repositions skin, fat and SMAS as a single unit.
Over the years, the tissues of the face descend vertically. That is why, in Dr. Fernando Mattioli’s planning, repositioning is also vertical, following the direction in which the tissues moved.

The indication is always individual and defined at the consultation. In general, a deep plane facelift is discussed in the following situations:
- Descent of the midface and cheek tissues
- Loss of definition of the jawline
- Deepening of the folds around the mouth related to tissue descent
- Planning face and neck in the same surgery, when indicated

Release of the deep plane
The surgeon accesses the plane beneath the SMAS and releases the ligaments that hold the tissues of the face.

Vertical repositioning
Skin, fat and SMAS are repositioned together, vertically, according to the plan defined at the consultation.

Combined steps
When indicated, the facelift is planned together with treatment of the neck, eyelids or forehead.
